Information in an enquiry or referral

The enquiry form asks for your name, email address, optional telephone number, your relationship to the enquiry, the type of support and a short message. The referral form also asks for your role and organisation, and non-identifying information about the learner, such as their key stage, general area, EHCP status, the reason for referral and the support requested. Please don’t include a learner’s name, date of birth, address, school, medical records or other identifying or sensitive information.

How this website uses the details

Your details are used to pass your enquiry or referral to the organisation and allow a response. They are passed to the notification or email service configured by the website host so the team can receive them. Where acknowledgement email is configured, that service also sends you a confirmation. Your details are not displayed publicly.

Technical information

Hosting and email services may process technical information needed to deliver and protect their services. The forms apply request limits to reduce spam. This website does not set optional advertising or analytics cookies. Typefaces are loaded from Google Fonts, which receives the technical information needed to serve those font files.

Choices and questions

You may contact us to ask about access, correction or deletion of your details, or to raise a concern about their use. You can also contact the Information Commissioner’s Office at ico.org.uk (opens in a new tab).
